Guidelines for Your Upcoming Session
Once an intake appointment is scheduled, you will be required to complete the necessary paperwork, including an intake questionniaire, consent forms, and payment information.
Policies Regarding Appointment Changes
Cancellations must be made 24 hours in advance or a fee of $100 will be collected.
Details on Payment Expectations
Connect Psychological Services (Connect) is currently a self-pay only practice. Connect does not accept third party payments. The entire balance is to be paid by the client as outlined in the contract for services. If applicable, we can provide you with a coded super bill showing paid in full after sessions or after the closing conference (testing) so that you can submit to your insurance provider to request reimbursement. Please reach out to your insurance provider if this might be an option.
